Sponsorship Approval Form for PDGA Major and Elite Events

Any PDGA Major or Elite event considering a sponsorship with a company in an industry listed in the “Exclusions” section of the PDGA Sponsorship and Advertising policy must complete this form for each potential sponsor. Please provide as much detail as possible when completing the form below. Lack of information can result in a delay of approval process. 

Written approval from all other stakeholders, approving of all details of this request, must be provided. Examples of such stakeholders are venue owner, tour or series manager, contracted media companies, government entities, etc.

All sponsorship requests must be submitted no later than 3 weeks prior to the start of the event to allow for proper review.

Please describe in detail what the sponsor is offering to the event (payment, merchandise, services-in-kind, etc) and what the sponsor is expecting in return.
If the sponsor will have a physical presence at the event, please describe to what extent. Examples: logo displayed on tee signs, caddy book, website, social media, etc. vending spaces, feather banners, wall-signs, product distribution or coupons
